Evaluation of the hemoglobin a1c test in detecting pediatric prediabetes

Aim: It was aimed to evaluate the diagnostic performance of hemoglobin A1c (HbA1c) test in pediatric prediabetes, and determine cut-off value for adolescent. 
 Material Method: This study carried out by retrospectively evaluating data 379 adolescents aged between 10 18 years. Prediabetes diagnosed based on glucose criteria, either fasting or 2-hour (2h) during a 75 g oral tolerance (OGTT), HbA1c criteria. The sensitivity, specificity, positive predictive (PPV), negative (NPV) were calculated fasting, OGTT 2h glucose, HbA1c. area under curve (AUC) each receiver-operating characteristic analysis. Results: 2.1% individuals assigned diabetes mellitus group, 21.9% prediabetes 76.0% normoglycemia group. When 5.7 used as threshold sensitivity 53.0%. AUC 0.83 test. An 5.55% determined optimal diagnosing with 62.7% 93.0% specificity. Conclusion: Although use adults’ criteria diagnosis ages is controversial due differences results HbA1c-based tests, screening still important. HbA1c≥5.55 will be useful follow up terms risk screen them blood glucose.
